      In the parish of the settlement of Miedzna, the eleventh / twenty-third of May eighteen ninty six at eleven o'clock, the witnesses Pawel S....., thirty-five [?] years [old], and Franciszek And...czyk, fifty years [old] (...) [came to] the parish church in the settlement of Miedzna and declared that on the ninth / twenty-first of May this year at eleven o'clock in the evening in the settlement of Miedzna, [there died] Maciej Oltusek, sixty two years [old], son of Ignacy and Jadwiga of Gorczyca Oltusek, widower [after the death] of Katarzyna born as Boryszewka (...)
