Note: Information received from the Special Collection of the Latter Day Saints in Salt Lake City. Bertha died at the hand of her husband. Cause of death is given as "Carotid Artery severed. The death certificate indicates that she was born 28 Dec. 1875 and that her father was Charles Fullick. However, her birth certificate gives her birth at 19 May 1875 and lists no father's name. In the 1881 census for Yorkshire, Bertha is listed as Bertha Simpson and it is assumed that she was illegitimate. In December 1875 her mother married Charles Albert Fullick and when the family came to the US from England, Bertha assumed their name.
Birth certificate is for Bertha Simpson, 19 May 1975, at 11 Errington St., Brotton, Yorkshire, registered on the 17th of August 1875. The certificate carries both the signature of the registrar and the superintendent of registrars. Presumably the signature of the superintendent was required because the birth was registered more than six weeks following the birth.
