a. Note:   Lee was diagnosed with angina within a year of Tom's death....
  When he had surgery for gallstones, his surgeon discounted the angina and said he had had gall bladder trouble all along. Lee was not even hooked to a heart monitor after surgery. The heart specialist was called in too late to help him the next day.
