In it he stated that he was 42 years old, born 25 Jul 1876 (error), and that he was a citizen of the united states by birth. He was a merchant by profession, living and working at #15 Calle Baldoriote, Juncos, Puerto Rico. His next of kin was listed as his sister Reparada Falero living at #13 Calle Celia Aguilas, Juncos, Puerto Rico. He was described as tall, slim, green eyes, black hair, no distinguishing marks.
