a. Note:   Family History: Obtained from the patient November 23, 1915. Dr. O'Neil. Father died at the age of 76 of tuberculosis. Mother died when about 77 of general debility. Brothers and sisters - Martin, age 60, living; George, dead, cause and age unknown; Frank, living at 55; Margaret, dead at age 40, cause unknownl Alice, a widow, 50 living and Mary about 45 or 50, single. One aunt had epilepsy and an uncle had tuberculosis. Patient was born in Wayne township, Marion County, Indiana August 26, 1835. He had the usual diseases of childhood. Started to school at the age of six years, never went much, only about two years. Then he went to work on the farm. When 18 years of age he worked out, was drafted into the Army at the age of 18 or 21, drafted twice. States he has been in the Army three times.
