Note: DEATH CERTIFICATE: Be It Remembered, That on this day to wit: the Twenty-Eighth of July in the year of our Lord, One Thousand, Nine Hundred and Thirty-four and the One Hundred and 59 of the Independence of the United States of America, before me, Wm. H. Robin, M. D., Chairman of the Board of Health and Ex-Officio Recorder of Births, Deaths and Marriages, in and for the Parish of Orleans, and the City of New Orleans, personally appeared Chas Letellier, an undertaker native of this city residing at No 2305 Cana St. who hereby declares that Vesta Smith Rehm (White) a native of N.O., LA aged 30 Yrs. departed this life Yesterday (27th of July 1934) at the Hotel Dieu (a hospital in N.O.) in this City, Resided at No. 1238 N. Broad St. Cause of Death Chr Nephritis, Secondary Anemia, Auricular Flutter. Certificate of Dr. P.H. Jones. Deceased was married to Daniel Rehm, and the Daughter of James a. Smith and Mary Farrell, both native of La. Thus done at New Orleans, in the presence of the aforesaid Chas. Letellier an also in that of P.H. Lanauze and W. J. Prudhomme both of this city witnesses by me requested here to be, who have hereunto set their hands together with me, after reading hereof this day, month and year first above written.
