a. Note:   Information found at the funeral home in Madison says that he was born in Blairstown, Indiana. However, there apparently is no such place.
 1880 census says he was born in Indiana. His obituary gives birthplace
 as Indiana. The register of his marriage on 22 Jan 1896 says he was
 born in Indiana and was age 24.
b. Note:   Cause of death was infarction of myocardium - coronary sclerosis.
c. Note:   They were married at the home of the pastor, Rev. A.C. Martyn of the Presbyterian church.
