a. Note:   George Vannoy died from complications from freezing while crossing the Ohio river in a blizard. He had walked from Ohio to Kentucky across the frozen river and was returning home from work. It is said that he suffered frost bite and that his nose, ears, lips, eyelids, fingers, and toes all fell off and that he died a very painful death after about six months of suffering.
