a. Note:   in St. Mary's Church, Derby, on June 26, 1870 by the Rev. P.J. Dwyer. The sponsors were Thomas McDermott and Catherine McGowan. The attending physician at the birth was Dr. Charles Phinney.
  John Joseph Boylan died on September 22, 1873 of "scarletina," according to his death certificate prepared by a Dr. Shelton. He is buried in an unmarked grave in Old St. Mary's Cemetery, now in Ansonia, Connecticut.
Note:   John Joseph Boylan was born on June 21, 1870 in Derby, Connecticut and baptized
