FORMBY: Donald Exton Formby, 71, Passed away Monday In Tucson, Ariz,. Mr. Formby is survived by his wife, Ann McCurdv of Tucson; two sons, Carl W. Formby of Tucson and Chuck W. Formby of Cedar Edge, Colo. and a sister, Lennis Sybert of, New Orleans, La. He had been the administrator at F ort Bayard Veteran’s Hospital prior to 1965. Services will be held Thursday at the memorial center at Fort Bayard National Cemetery at 2 p.m. The Rev. C. M. Henderson wil1 officiate. Arrangements are being made through the Curtis Bright Funeral Home of Silver City.
