State of Kentucky, USA Cumberland County On this 14th day of February 1894 personally appeared before me, a County Clerk in and for Cumberland County, GEORGE W. BROCKMAN aged 26 years and KENLEY C. DERTING aged 39 years who being by me duly sworn depose and say that we were well and personally acquainted with THOMAS COLLINS, deceased, who was late Private Co C 13 Kentucky, USACalvery soldiers and also his widow ANGELETTA COLLINS, who still remaining his widow and lives near Amandaville, Cumberland, Kentucky, USA. They state that said THOMAS COLLINS died in Cumberland County, Kentucky, USAon the 3rd day of January 1894 being killed by a team of horses running away with him. They state that said COLLINS died the owner of a small track of land worth some five or six hundred dollars, some horses, and some few hogs and cattle, with thirty or forty dollars. They stat that said COLLINS left five children as his heirs at law. That is all the property both real, personal and mixed left by the said COLLINS except a few household goods. We make these statements from a personal knowledge of the facts having no interest in this matter. Our Post Office is Amandaville, Cumberland, Kentucky, USA /s/ GEORGE W. BROCKMAN and KENLEY C.
