Special Minimum Wage Permit
Pursuant to Section 1191, California Labor Code, and Section 6 of Industrial Welfare Commission Order 1-68, Hillside Enterprises, 4525 E. Stearns, Long Beach, is hereby authorized to employ Terry L. Andrus at the special minimum wage of 15 cents per hour plus for not more than 6 hours per day and 5 days per week.
All other provisions of Industrial Welfare Commission Order 1-68 shall be observed. Permit Number 71-339 Date Issued 3/1/71 Void after 2/29/72 or piece rates based on $1.65 an hour to non handicapped workers, whichever is higher. ...

Dear Mrs. Andrus:
Terry seems to be adjusting to our program splendidly and we feel sure that she will make a fine addition to our membership. She tells me she is enjoying the group.
It will be necessary for you to come in for a personal interview (at your convenience), fill out the forms and we will be “in business”.
The application is passed upon by our board at their next meeting after filing, which in this case will be in January. there is a six month probationary period, but I feel quite sure we will have no problems. Terry is a beautifully mannered girl and really fits in well with our present group. Please call me at your earliest convenience and we will set up the time for interview; at that time we will discuss any medication, special problems if any, and her emergency doctor etc.
