a. Note:   William Terrell Lewis author of Genealogy of the Lewis Family got his information from a letter written by David Benjamin Lewis (born July 1820) dated February 15th 1856. On page two it says ".....my mother Martha Lewis died the 5th of Oct 1826 in the 44th year of her age, her maiden name was Baker, her father's name was Glover Baker of Liberty County, Va. [It was actually the town of Liberty, Bedford Co., Virginia] & this is all that I know about my mother & Grand Father on her side. ....The original document is on file at the Alabama Archives in Montgomery, Alabama, in the William Terrell Lewis Collection, boxes 121 & 122." In the letter David Benjamin Lewis also listed the names and birth dates of his siblings, along with some of their marriages and death dates.
  Genealogy of the Lewis Family in America, page 334: Captain David Benjamin Lewis, son of David J., was born in 1820 in Breckinridge county, Kentucky, soon after his father settled in said county. He is six feet in stature, weighing one hundred and seventy pounds, with fair skin, blue eyes, dark hair and of sanguine temperment. He is a farmer; sometimes taught school and the latter part of his life he practiced medicine. He resides near Howell Springs, in Hardin county, Ky. He is a steward in the Methodist church and captain of the militia in his county. In 1839 he married Lucy Moorman, daughter of Achilles Moorman, of Hardin county, Kentucky. She was first cousin to Elizabeth Moorman, the first wife of Dr. Jesse P. Lewis.
