a. Note:   The Guzinski name is Polish, with Milwaukee the home of several Guzinski families (Gazinski, Gesinski, Gaginski spellings) in 1900. Since Thomas was born in Dec. 1900, he would not appear in the 1900 census. He first appears in th 1910 census with his family as a 10 year old, born in Wisconsin.
 The 1920 Census, taken Jan. 1920 ,shows Thomas Sylvan Guzinski, Seaman 2nd Class, single, age 19 years, born Wisconsin, mother and father's birthplace Poland. He was in the Navy onboard the ship "USS Eagle 46", out of Portsmouth, New Hampshire. His home address was 919 Cut__ski St. Milwaukee, WI.
 The 1925-27 (Oct. 1926) City Directory of Newburyport, MA shows Thomas S. and Alberta Guzinski, shoeworker, at 26 Madison St., Newburyport. About 1929 (he is not in the 1930 census of either Milwaukee, WI or Newburyport, MA), Thomas, Alberta and their children moved to Milwaukee. The 1929 City Directory of Millwaukee shows the couple at 513 Cramer St. Within a couple of years Alberta left her husband and moved back to Newburyport with their children, living with an Aunt and Uncle who had raised her.
 In 1940 Thomas was living with his mother, brother Frank and sister Margaret.
 Military Records show that Thomas S. Guzinski, born 1900 in Wisconsin, enlisted in the Army on October 9, 1942 in Wisconsin as a Private (Branch, Warrant Officer, USA). His civilian occupation was "Laboratory Technician, or Motion Picture or Shipping Clerk". His marital status was "separated, with dependents". Height 66 inches, Weight 205 Lbs. He served until March 18, 1943.
 The Social Security Death Index shows his last known address was 53221 Milwaukee, Milwaukee, Wisconsin, and his death certificate shows 1511A West Morgan Ave, Milwaukee.
