Notes
a. Note:   She remembers working in cotton fields and picking fruit as a little girl as the family migrated between CA and OK. She has worked at Gatesway Foundation in Broken Arrow, OK since 1987.
Mom died of a massive heart attack, beginning about 11:30pm on 05 NOV 2012. She called my brother Doy to drive her to the hospital. As they were getting onto US412 at the E 289th Street exit, she gasped and slumped onto Doy. He, along with my niece Checotah who was supporting Mom, drove at speeds of 110 mph through a constuction zone, knocking over safety barrels in the process, until he reached the Quicktirp store in Catoosa where he performed CPR until the ambulance arrived. She was shocked with chest paddles and CPR was continued until she got to St. Francis Hospital in Tulsa. She had faint signs of a pulse, but was declared dead at 12:24 AM on 06 NOV 2012. I arrived from Edmond around 3 AM and saw her there.
In December 2012 Marci and I were going through her papers and recovered a notebook she kept by her bed. In that notebook, she had written about 2 hours before she died that she had taken an insulin shot due to a high blood sugar spike.
