Colee aged 52 years, a resident of Noble Tp. in the County of Shelby and State of Indiana whose Post office address is St. Paul, Ind. and Joseph Mitchell aged 51 years a resident of Noble Tp. in the County of Shelby and State of Indiana whost Post Office address is St. Paul, Ind...declare...That they have been well and personally adquainted with George W Hernden, dec. for 25 years, and 25 years respectively, and that said Soldier was the support of the Father and Mothe and Brothers and Sisters of himself, that said Soldier sent foty [sic] dollar to said family that the Mother Elizabeth Hernden, receied said sum before she died in Oct 1862, and that subsequet to that he sent diffent sums twice, to his youngest Brother Joseph N Hernden, who was about Three year old at the time, This in the years 1863&4...The matters and [unclear] set forth above are true as they firmly believe...[signed] Juleius L. In the matter of Brothers and Sisters of George W Herndon, dec, who was a Corporal...Jerome Copeland...and Nancy O Copland...declare...That we were neighbors of the aforesaid Parties, (mistake) the Parents of said Soldier, (mistake) George W Herndon, dec...That we made a former affidavit as to being at the Wedding and Marriage of said Parents of sold...That we were very close neighbors and one affiant was present at the Birth of said deceased Soldier, who was Born on or about Oct 13th 1843, said child being a Legitimate heirr of William Herndon dec. and Elizabeth Herndon, dec. and a Brother to the Claimants...[signed] J. That said George W Herndon, was Born October 13th 1842, said George W Herndon, dec. being a Legitimate Child of William Herndon, dec. and Elizabeth Herndon, dec. That this affiant knows the above facts from being an uncle of the said George W. Herndon, deceased...[signed] John S. (Source 6) This form is different than another form dated 21 Aug 1893. We have noticed several items have been crossed out or struckthrough on it. Due to the fact we received a copy from NARA, it seems that this form had been submitted with another form bearing the same date, yet no action nor approval was taken on this form. JDS/MKF
